Implementation

Helen has a history of poor mental health.  She is currently feeling better than she has in some time, and has recently has started going to church and found comfort in this. She is friendly and chatty and will certainly benefit from the interaction with the team. She is happy to accept our support.   

Helen’s daughter, Lottie, is her main support. She lives nearby and has just had a baby so Helen spends some time there.   

Helen’s referral was from the social prescribing team, primarily to help alleviate her isolation.

Excerpts from the monitors diary

Bless her, she was expressing how lonely she is this morning. I said she won’t have time to be lonely now she is one of our service users! She is such a lovely lady and very compliant. She took her medication before any informal chat. I do feel she will really benefit from the service not just compliance but well being.

Susan has been very anxious through the night she thought someone had rang her doorbell and then her mind ran away with her – she talked about feeling isolated. Told me about being acrophobic and how she doesn’t go out. 

Susan was telling me about her accident and how she had to to give up her dog as she was unable to walk it after the accident. Drunk neighbours pushed her over and she damaged her back. She has asked if we can look into a befriending service for her.

We’ll make enquiries into Silverline or similar befriending service for Susan. Have provided 2 contact details for Susan if she would like to call. Silverline and BootleBuddies, on call list for Susan. – she has been to Bootle Buddies.
